We are in the process of formulating the information for our 2015 – 2016 Student Directory. In order to be in compliance with The Family Educational Rights and Privacy Act your permission is necessary for the school to publish information regarding you and/or your children. Please download and complete the form below if you wish to have your information published in the 2015 – 2016 Directory. Please complete and return the entire form to the school office on or before Friday, August 28, 2015. If we have not received your release by that date we will assume that you wish to be deleted from the Directory.
